Job Description:
A company is looking for a Care Management Support Coordinator
II to support administrative care management activities.
Key Responsibilities
Perform outreach to members and assist with care plan next steps
and resources
Serve as the front-line support for member and provider inquiries
and concerns
Document and maintain non-clinical member records in accordance
with regulations
Required Qualifications
High School diploma or GED
1 - 2 years of related experience
Knowledge of existing benefits and resources locally and make
referrals to address Social Determinants of Health (SDOH) needs
